BOX 187 <br /> Craig, CO 81626 <br /> 20 July 2021 <br /> Re: Trapper Mining, Inc., Permit C1981010 <br /> 2021 Quarter 2 Horse Gulch Fill Inspection Report <br /> Dear Ms. Perkins: <br /> The Colorado Division of Reclamation, Mining, and Safety (CDRMS) has reviewed and <br /> found the above referenced report to be in general compliance with the following CDRMS <br /> regulations: <br /> 4.09.1(11(a) Inspected at least quarterly by a registered engineer, or other qualified <br /> professional specialist in construction of earth and rockfall embankments under the direction <br /> of a qualified engineer throughout the construction. <br /> The certified report covers the second quarter monitoring period from March through June of <br /> 2021, and the inspection was conducted by yourself, a certified engineer on 9 June 2021. <br /> 4.09.1(11)(b) Provide to the Division a certified report within 2 weeks after each inspection <br /> that the fill has been constructed as specified in the design approved by the Division. The <br /> report shall include a description of any appearances of instability, structural weakness, and <br /> other hazardous conditions. <br /> The Division received the report via electronic transmission on 25 June 2021. No instability, <br /> structural weakness, or other hazardous conditions were noted during this inspection of the <br /> fill. The area where cracks were detected on the top south portion of the fill during the first <br /> quarter of 2013 was inspected and no additional cracking was observed. Other areas where <br /> differential settling has been observed in the past were also inspected.
