PERMIT#: M-2021-030 <br /> INSPECTOR'S INITIALS: ECS <br /> INSPECTION DATE: July 15,2021 <br /> OBSERVATIONS <br /> This was a pre-operation inspection for the WCR 40 Dirt Mine, a proposed 149 acre, 112c permit to be <br /> operated by David Hunt/Hunt Farms. The proposed permit is located at the SW corner of the intersection of <br /> WCR 40 and WCR 31. The site entrance is located on the south side of WCR 40 on the north side of the <br /> proposed permit area. DRMS representative Eric Scott met permitting consultant for the Operator, Andy <br /> Rodriguez of Civil Resources, at the site entrance at approximately 09:30 and toured the proposed permit <br /> area. The permit applied for notice was present at the site entrance as well as the site identification sign <br /> (photo 1). <br /> The site is currently in use as irrigated corn field. The conditions on-site have appear to have been accurately <br /> presented in the permit application, and representative photos of the pre-mining conditions have been <br /> included in this report. <br /> The only issue noted during this inspection was that the permit boundary, as currently marked by green <br /> flagging tape tied to the existing fences, does not match the permit boundary presented the application maps. <br /> This will need to be corrected and permanent markers will need to be placed in the proper locations as stated <br /> on the first page of this report. <br /> No other potential issues were identified during this inspection. <br /> Inspection Contact Address <br /> David Hunt <br /> Hunt Farms <br /> 14460 WCR 40 <br /> Platteville, CO 80651 <br /> Page 2 of 4 <br />
