Compliance With Mine Plan N - Support Facilities Not On-site <br /> N -Other N - Special Categories Of Mining <br /> N -Processing Waste R -Topsoil <br /> COMMENTS <br /> This was a partial inspection conducted by Lucas West of the Colorado Division of Reclamation,Mining and <br /> Safety as part of the normal monitoring program for permitted sites. No Operator representative was present <br /> during the inspection,Elk ridge Personnel were assisting repairs to the the Area water supplu company that <br /> suffered a major ditch failure. The site is in Final Reclamation and was not active the day of the inspection. the <br /> weather was clear and dry, Six photos accompanty this report to illustrate the observed site conditions. <br /> HYDROLOGIC BALANCE -Rule 4.05 <br /> Drainage Control 4.05.1,4.05.2,4.05.3; Siltation Structures 4.05.5,4.05.6; Discharge Structures 4.05.7,4.05.10; <br /> Diversions 4.05.4;Effluent Limits 4.05.2; Ground Water Monitoring 4.05.13; Surface Water Monitoring <br /> 4.05.13; Drainage —Acid and Toxic Materials 4.05.8;Impoundments 4.05.6,4.05.9; Stream Buffer Zones <br /> 4.05.18: <br /> Both Pond 001 and Pond 002 were observed,with no standing water as the area is experiencing <br /> significant drought conditions. The area around Pond 001 is well vegetated and appeared to be stable at the time <br /> of the inspection. No signs of erosion were observed on the in slope area. Pond 001 can be seen in Photo One. <br /> The inlet structure is in good condition and no signs of head cutting or downstream erosion was observed. The <br /> ditch leading to Pond 001 was also in good condition. The section of the liner of the ditch has been removed, and <br /> no head cutting at the transition point where the liner remains was noted. The transition point from unlined to <br /> lined ditch can be seen in Photo Two. <br /> Pond 002 is well vegetated and all side slopes were stable at the time of the inspection. The pond can be seen in <br /> Photo Three. Both inlet structures to the pond were in excellent condition and no evidence of recent flow was <br /> observed at either inlet point. The ditch leading to Pond 002 was clear and free from obstruction but also showed <br /> no signs of recent flows. <br /> The area water supply ditch failure has cut off water from the entire area including the reclaimed area that is <br /> irrigated crop land. The landowner has performed the first cutting of the year and active bailing and rolling <br /> operations were underway during the inspection. <br /> Number of Partial Inspection this Fiscal Year: 8 <br /> Number of Complete Inspections this Fiscal Year: 4 <br /> Page 2 of 6 <br />
