OURAY SILVER MINES <br /> 5.3 Waste Disposal <br /> Wastes resulting from a minor spill response will be containerized in impervious bags, drums or <br /> buckets. Materials that are reactive with each other will not be containerized together i.e., no <br /> oxidizers and organics together. The waste will be removed from the site by a licensed waste <br /> hauler within one month. <br /> Wastes resulting from a major spill response will be removed and disposed of by a cleanup <br /> contractor. <br /> 5.4 Spill Documentation and Notification <br /> If a spill occurs,the appropriate Spill Notification Form will be completed. Take photos of the <br /> spill and how it is contained.The Environmental Department will file the form in a central <br /> location in the Environmental Office. <br /> Any spill that affects or threatens to affect Sneffels Creek or adjoining shorelines must be <br /> reported immediately to the EPA National Response Center <br /> The person reporting the discharge must provide the following information: <br /> • Name, location, organization and telephone number; <br /> • Date and time of the incident; <br /> • Location of the incident; <br /> • Source and cause of the release or discharge; <br /> • Types of material(s) released or discharged; <br /> • Quantity of material(s) released or discharged; <br /> • Danger or threat posed by the release or discharge; <br /> • Number and types of injuries (if any); <br /> • Media affected or threatened by the discharge (land/air/water); <br /> • Weather conditions at the incident location; and <br /> • Any other information that may help emergency personnel respond to the incident. <br /> Spill Prevention Control and Countermeasure Plan 28 1 P a g e <br />
