May 7,2021 C-1980-007/West Elk Mine LDS <br /> D and S Underground,Inc. of Hotchkiss,CO,had been contracted and had begun mobilizing equipment to <br /> the site. Contractors were expected to be on site from the week beginning May 10 through mid-June. They were <br /> expected to begin by regrading and repairing roads, and refreshing sediment control structures,before proceeding <br /> with the reclamation of several pads on the E6,7 and 8 longwall panels. <br /> GENERAL MINE PLAN COMPLIANCE: <br /> The longwall was in the SS panel,at around cross-cut 12 on the SS head gate. <br /> Development of the SS3 head gate had progressed to around cross-cut 5, and development of the Sunset 4 <br /> Submains had just begun. <br /> RECLAMATION SUCCESS -Rule 4.15,Rule 3: <br /> A group of monitoring wells had been included in the SL-12 Partial Phase I bond release application,but <br /> were overlooked during the bond release inspection. Their locations were verified and photographed. Survey pins <br /> were located, and satisfactory reclamation was verified,for the following holes: <br /> SW-1 <br /> SOM-2 <br /> SOM-13 <br /> SOM-16 <br /> SOM-23H1 <br /> SOM-23H2 <br /> SOM-23H3 <br /> SOM-23H4 <br /> SOM-45H 1* <br /> *A second,un-reclaimed,PVC-cased hole was found at the same location as the survey pin marked <br /> "SOM-45H1". It appears that the open hole is actually SOM-45H1,which is still being monitored, and that the <br /> plugged and abandoned hole next to it was SOM-45H2 (and was labelled incorrectly on the survey pin—see figure <br /> 9). <br /> SUPPORT FACILITIES -Rule 4.04: <br /> The mobile flare was in place on SS1-6,but was not operating at the time of the inspection since the <br /> partial pressure of methane in the mine was below the flare's operational range. <br /> The road accessing the SS2 MVBs was secure behind a locked gate, and it was clear that it had not been <br /> travelled recently. The SS2-3 and-5 pads were inspected from the SS1 road.
