The annual fee required for your pern-ift is $323.00. As of the date of this letter the <br /> Division has not received your submittal. <br /> Therefore,you are in violation of the above cited statute and the provisions of your Permit No.M-2007. <br /> 050. Pursuant to C.R.S. 34-32.5-124, the Mined Land Reclamation Board hereby assesses Notice of <br /> Violation MV-2021-009, Civil Penalties in the amount of$323.00, and an Order to Cease and Desist <br /> from all mining related activities associated with Permit No. M-2007-050. <br /> Pursuant to C.R.S. 34-32.5-116(3)(b), a Formal Public Hearing before the Mined Land Reclamation <br /> Board(the"Board")has been scheduled for June 23 and 24,2021 Board Meeting. The hearing will <br /> be for the consideration of permit revocation and finding the financial warranty subject to forfeiture. <br /> Due to the impacts of the COVID-1 9 virus on availability of State facilities and Division resources, <br /> notice is hereby given the June 23 and 24, 2021 Board hearing will be by a virtual ZOOM hearing. <br /> In order to participate in the hearing, all exhibits and materials you wish for the Board to consider <br /> must be submitted by June 11,2021 via email to the MLRB Administrator. You must also provide an <br /> email of all parties to the MLRB Administrator,Camic Mojar at: Camille,nlqiqrs' state,co,us, no later <br /> than 5:00 PM,June 11,2021 in order to receive an authorization code to attend or to request a <br /> continuance of the matter.
