Overnight Priority Delivery and Email to Peter.hays(&state.co.its Division of Reclamation, Mining and Safety c/o Peter Hays RECEIVED 1313 Sherman Street, Room 215 Denver, CO 80203 MAY 0 5 2021 Re: Submittal in Response to April 30, 2021 Incompleteness Letter DIVISION OF RECLAMATION Amendment to Existing Permit M-1983-033 Walstrum Quarry MININGAND SAFEW Dear Peter: Enclosed please find the items needed to address the April 30, 2021 Incompleteness Letter: • One (1) original and one (1) copy of the completed Regular 112 Operation Application Form that reflects the company name as "Albert Frei & Sons, Inc." by adding a period at the end and re-doing the handwritten notary acknowledgment to replace the "and" symbol with the "&" symbol; and • Two (2) copies of a revised Exhibit R Proof of Filing with County Clerk and Recorder dated May 4,2021 to reflect the revised application form above and documents below have been replaced in the copy on file with the Clerk and Recorder. Given the foregoing, we took this opportunity to update and enclose the following items, which replace the previously submitted items in their entirety as indicated below: • Two (2) copies of revised Exhibit D that has a handful of minor wording changes; • Two (2) copies of revised Exhibit F (Sheet F-1 only) that has one minor change, the reclamation type color has been added to the Texaco Placer, Parcel C2; and • Two(2)copies of revised Exhibit S cover sheet that references the counter-signed Structure Agreement for CDOT received after initial submittal and previously submitted to DRMS on April 28, 2021.
