Leach pad staff responded quickly, turned off valves, cleaned- <br /> up off liner cyanide affected soil, and applied calcium hypochlorite to <br /> mitigate any remaining cyanide. <br /> c. Upon <br /> reaching pressure equalization between the two pipes, the structural capacity of the 12 <br /> inch HDPE pipe was exceeded and cracked and then broke at the factory weld. <br />
