22 March 2021 C-1981-044/Williams Fork Mines RAR <br /> ROADS Rule 4.03 <br /> Construction 4.03.1(3)14.03.2(3);Drainage 4.03.1(4) I4.03.2(4); Surfacing and Maintenance <br /> 4.03.1(5) and(6)14.03.2(5) and(6);Reclamation 4.03.1(7) 14.03.2(7) <br /> o The Haul Road was dry, in good repair and well maintained at the time of inspection. <br /> o Access roads around the site appeared in good repair with no ruts or standing water. <br /> o Ditches appeared to be functioning adequately. <br /> HYDROLOGIC BALANCE Rule 4.05 <br /> Drainage Control 4.05.1, 4.05.2, 4.05.3; Siltation Structures 4.05.5, 4.05.6;Discharge <br /> Structures 4.05.7, 4.05.10;Diversions 4.05.4;Effluent Limits 4.05.2; Ground Water Monitoring <br /> 4.05.13; Drainage-Acid and Toxic Materials 4.05.8;Impoundments 4.05.6,4.05.9; Stream <br /> Buffer Zones 4.05.18 <br /> o Unless otherwise noted,pond banks and dams appeared stable at the time of inspection. <br /> o None of the ponds held water at the time of inspection. <br /> o Ditches were dry, and functioning adequately. <br /> o A ditch along the north end of the refuse pile directs runoff to the sediment pond below. <br /> TOPSOIL RULE 4.06 <br /> Removal 4.06.2; Substitute Materials 4.06.4(4); Storage and Protection 4.06.3;Redistribution <br /> 4.06.4 <br /> o No instability, rilling or gullying were noted on the observed topsoil piles. <br /> o Piles appeared to be marked appropriately with tposts. <br /> BACKFILL and GRADING R ULE 4.14 <br /> o No backfilling and grading occurred at the portal during the 2020 filed season. These <br /> activities are scheduled to resume in spring 2021. <br /> o Refuse pile has been topsoiled as of 12 Dec 2021. <br /> RE VEGETATION-Rule 4.15 <br /> General Requirements 4.15.1; Use of Species 4.15.2, Grazing 4.15.5 <br /> o Reclamation and revegetation appeared to be progressing adequately. <br /> o Seeding that occurred in fall of 2019 and 2020 along the bottomlands of the Williams <br /> Fork River appeared to be emerging. <br /> o Vegetation throughout the site remained dormant throughout the site. <br /> DOCUMENTS RECEIVED <br /> None <br /> ENFORCEMENT ACTIONS/COMPLIANCE <br /> No enforcement actions were initiated as a result of this inspection, nor are any pending. <br /> Number of Partial Inspection this Fiscal Year: 7 <br /> Number of Complete Inspections this Fiscal Year: 3 <br />
