Preliminary review of the information received determined that the additional <br /> information must be received before the Division can consider your application as being submitted <br /> and technical review can begin. <br /> Exhibit E: Rule 6.2.1 (2)(a) Please provide Maps showing the Name of the Applicant. <br /> Rule 6.2.1(2)(d) Identify and outline the area which corresponds with the application. <br /> Rule 6.2.1(2) (e) Please provide an appropriate legend on Maps submitted. <br /> Exhibit L: Please submit structure agreement for the irrigation ditch that is within the permit <br /> boundary and the USFS road used for Access. <br /> Where the mining operation will adversely affect the stability of any significant, valuable and <br /> permanent man-made structure located within two hundred (200) feet of the affected land, <br /> the Applicant shall either: <br /> (a) provide a notarized agreement between the Applicant and the Person(s) having an <br /> interest in the structure, that the Applicant is to provide compensation for any damage to <br /> the structure; or <br /> (b) where such an agreement cannot be reached, the Applicant shall provide an appropriate <br /> engineering evaluation that demonstrates that such structure shall not be damaged by <br /> activities occurring at the mining operation; or <br /> (c) where such structure is a utility, the applicant may supply a notarized letter, on utility <br /> letterhead, from the owner(s) of the utility that the mining and reclamation activities, as <br /> proposed, will have "no negative effect" on their utility. <br /> c ~ <br /> �c��°�qo� <br /> I 3 5htrrn:;.,5t- P.r%o-11 115[1rnVer, CO 80-/03 F, (M)86-5-3567 F (3031832-8106 h Ups:rl co;ore r rc i:,cv,d-rn_ <br /> .i,rrz'.1 FJ.IS, Go'.err Da-r GIL'tas.
