Frank Ferris and Mindy Sickels of Elk Ridge <br />Mining and Reclamaiton accompanied the inspection and represented the Operator. The site is in final relcmation <br />and was not active the day of the inspection. The weather was clear and dry. Five Photos accompany this report <br />to illustrate site conditions. <br />HYDROLOGIC BALANCE - Rule 4.05 <br />Drainage Control 4.05.1, 4.05.2, 4.05.3; Siltation Structures 4.05.5, 4.05.6; Discharge Structures 4.05.7, 4.05.10; <br />Diversions 4.05.4; Effluent Limits 4.05.2; Ground Water Monitoring 4.05.13; Surface Water Monitoring 4.05.13; <br />Drainage — Acid and Toxic Materials 4.05.8; Impoundments 4.05.6, 4.05.9; Stream Buffer Zones 4.05.18: <br />Both Pond No. 1 and Pond No. 2 were dry at the time of the inspection. The ponds can be seen in Photos <br />One and Two. All inslopes leading into the ponds appeared stable and in good condition. No evidence of <br />significant erosion nor formation of rills or gullies was noted in either pond. The inlet structure to Pond No. 1 was <br />observed, no signs of head cutting or erosion was observed. During the Division's November 2020 partial <br />inspection minor damage was noted to the inlet of Pond No. 2. All damage has been repaired and the inlet <br />structure and its liner are in good condition. <br />The western ditch leading into Pond No. 1 is in excellent condition. The liner is intact with no damage at this <br />time. Discussions with the Operator indicate the removal of the liner is imminent in accordance with their permit. <br />All other ditches leading to both ponds were in excellent condition and free from obstructions. Examples of the <br />ditches can be seen in Photos Three and Four. <br />SUPPORT FACILITIES - Rule 4.04: <br />The facilities area located towards the south end of the site was observed as seen in Photo Five. The area <br />was in good condition and was stable at the time of the inspection.
