A coPy of this <br /> "Notice of Filing Application"form is attached for your use. <br /> 2.If the mining operation is within the boundaries of a conservation district.you MUST send a notice.on a form approved b" <br /> the Board.to the board of'supervisors of the conservation district.A copy of this"Notice of filing Application"form is <br /> attached for your use. <br /> 3.You MUST include proof of notice of#1 and#2 at the time the application is submitted to the Office for filing(Rule <br /> I.6.2(1)(g)). <br /> a.You MUST place a copy of the application.less confidential items.foi public review with the clerk and recorder of the <br /> countN or counties in which the affected land is located.You MUST provide the office with an affidavit or receipt indicating <br /> the date on which the copy of the application was placed with the clerk and recorder at the time the application is submitted to <br /> the Office for filing(Rule 1.6.20)(0). <br /> 5.Am changes or additions made to an application submittal MUST be filed with the count,� clerk and recorder.
