Release Requests for Construction Materials Notices of Intent (NOIs) <br /> A request for release from reclamation liability for a Construction Materials Notice of Intent <br /> must include the following [pursuant to Rules 4.16.1(2)]: <br /> ➢ File information, including: <br /> o name of the operation <br /> o file number of the Exploration Notice of Intent, <br /> o name of the Person conducting exploration, and <br /> o name, mailing address and phone number of the contact person <br /> ➢ A signed statement by the Person conducting exploration that all reclamation..r-*� <br /> requirements of the Exploration Notice have been satisfied; <br /> ➢ A narrative describing site grading, topsoil replacement, successful revegetation and <br /> other stabilization activities, as appropriate; <br /> ➢ Photographs of the reclaimed area demonstrating competed reclamation; and <br /> ➢ A map of sufficient detail to determine the location of the exploration activities such as <br /> bore holes, test pits,monitoring holes, shafts and tunnels. <br /> In addition, if not already provided to the Division, demonstration of the proper abandonment of <br /> all exploration drill and monitoring holes must be provided in accordance with Rules 5.4.2, 5.4.4 <br /> or 5.4.5 (see reference Rules below). <br /> Reference Rules: <br /> Rule 4.16.1: Operator Application for Release of Warranties <br /> �((1) Upon the completion of reclamation, any person that filed with the Board or Office an Exploration <br /> / Notice of Intent and Financial Warranties shall submit to the Office by Certified Mail and separate <br /> from other types of communication to the Board or Office a Reclamation Report and request for <br /> reclamation responsibility release stating that reclamation is finished. <br /> (2) Such report shall contain, at a minimum: <br /> ?C (a)the name of the operation, the name of the Person conducting exploration, file number of the77- <br /> Exploration Notice of Intent and the name, mailing address and phone number of the contact <br /> person; <br /> }� (b) a signed statement by the Person conducting exploration that all reclamation requirements of <br /> the Exploration Notice have been satisfied; <br /> /(c) a narrative describing site grading, topsoil replacement, successful revegetation and other <br /> �` stabilization activities,as appropriate; <br /> X-(d)suitable photographs of the reclaimed area; and <br /> �((e)a map of sufficient detail to determine the location of the exploration activity. <br /> Rule\5.3.5: Post-Reclamation-Inspection and Release of Warranties <br /> (1) The Office shall inspect the lands explored within ninety (90) days after the Person conducting <br /> exploration on the lands submits a Reclamation Report and request for reclamation responsibility <br /> release, meeting the requirements of Rule 4.16.1(2), including permanent abandonment of all <br /> exploration drill holes as required by Rule 5.4.2 or 5.4.5. If the Office finds the reclamation <br /> satisfactory,the Office shall release the applicable Performance and Financial Warranties. <br /> Page 1 <br />
