COLORADO LEGACY LAND <br /> FORMER$CHWARTZWALDER MINE <br /> • 'Regulatory Agencies"for this project are the Colorado Department of Public Health and Environment <br /> Radiation Program and the Colorado Division of Reclamation, Mining, and Safety (DRMS). These <br /> agencies are responsible for ensuring that the public health and the environment are protected. DRMS <br /> administers the Mine Reclamation Permit (No. M-19977-300), which permitted the construction of <br /> this diversion channel under Technical Revision#28. <br /> • "Prime Contractor" was responsible for executing the activities specified in the Construction Quality <br /> Assurance/ Quality Control (QA/QC) Plan (Ensero, 2020) and construction oversite to ensure that <br /> quality standards specified by the design documents were met. Ensero acted in this capacity for the <br /> project. <br /> o "Engineer"is a representative of Ensero,Sam Lowe P.E.a registered Professional Engineer in <br /> the State of Colorado. <br /> o "Construction Quality Manager"was responsible for the execution and oversight of this plan, <br /> including the verification that the Project is executed in accordance with the Project <br /> specifications as defined in Technical Revision #28. Billy Ray acted in the capacity of the <br /> Construction Quality Manager for Ensero. <br /> o "Project Manager" was responsible for the organization and reporting of Project <br /> documentation to ensure appropriate quality records are maintained and retained. Elizabeth <br /> Busby acted in the capacity of the Project Manager for Ensero. <br /> • "Construction Subcontractor" was responsible for executing the Project excavation, earthwork, <br /> construction,and concrete work. Kessler Reclamation acted in this capacity for the project. <br /> • "Installation Subcontractor' was responsible for geosynthetic-related construction activities. H&H <br /> Lining acted in this capacity for the project. <br /> • "Surveying Subcontractor' was responsible for identifying local control points, setting construction <br /> staking,and performing a final(as-built)survey of the channel. Lambert Land Consulting acted in this <br /> capacity for the project. <br /> 1.2 STANDARDS AND TEST METHODS <br /> All tests performed were in accordance with the approved Technical Specifications referenced in the <br /> Construction QA/QC Plan (Ensero, 2020) approved with Technical Revision #28. These specifications are <br /> included in Appendix F of this Report. <br /> 1.3 CONSTRUCTION DRAWINGS AND TECHNICAL SPECIFICATIONS <br /> The requirements for construction,testing frequency and test methods were referenced from the Construction <br /> QA/QC Plan (Ensero, 2020), Construction Drawings, and Technical Specifications identified in Technical <br /> Revision#28. These construction drawings and technical specifications are included herein as Appendix F and <br /> Appendix G,respectively. <br /> DECEMBER 2020 2 NWRP CHANNEL RECORD OF CONSTRUCTION <br />
