3. If Operator fails to comply with any of the corrective actions of this Order <br />within thirty days of the effective date of this Order, the Division is <br />directed to schedule the matter before the Board for permit revocation at <br />the Board's next available meeting. <br />The Board imposes a CIVIL PENALTY for 79 days of violation at $1000 per <br />day for a total civil penalty of $79,000.00, with all but $10,000 of the penalty <br />suspended if Operator complies with the corrective actions. The $10,000.00 in <br />unsuspended civil penalties is due and payable within 30 days of the effective date <br />of this Order. Where corrective actions are not completed within the required <br />period, the unsuspended portion of the civil. penalties, $69,000.00, shall be <br />immediately due and payable.
