PERMIT #: M-2018-037 <br />INSPECTOR’S INITIALS: ECS <br />INSPECTION DATE: August 12, 2020 <br /> <br />Page 2 of 3 <br /> OBSERVATIONS <br /> <br />This was a pre-operation inspection for the proposed 112c Inouye Gravel Mine Amendment AM 01. The site was <br />inspected by DRMS representative Eric Scott who was accompanied by permittee representative Mark Johnson, <br />permitting consultant Andy Rodriguez, and representatives of the Ft. Lupton Wastewater Treatment Plant. The proposed <br />58.9 acre amendment area is accessed from the north side of Highway 52 approximately ¼ mile west of the intersection <br />of Hwy 52 and Hwy 85 , via the existing Ft. Lupton wastewater treatment plant access road. The proposed amendment <br />area is part of the Ft Lupton wastewater treatment plant. <br /> <br />As shown in the attached photo, the permit applied for notice was clearly posted at the access gate to the wastewater <br />treatement plant , and the proposed permit boundary was marked with lath and pink flagging tape, with the exception of <br />the areas where the parcel boundary is located within the river channel. Although the current ly marked amendment area <br />includes all of the Ft Lupton wastewater treatment plant parcel, it should be noted that mining activity as proposed in the <br />amendment will be restricted almost entirely to the area currently within the chain-link fence, and north of the treatment <br />plant , in the existing overflow storage areas. The existing overflow excavations located on the north side of the treatment <br />plant will be lined with a slurry wall and the enclosed area will be further mined for additional storage capacity. Any <br />deviations from the mining plan as proposed in the amendment will need to be approved by DRMS through the revision <br />process. <br /> <br />All structures observed during the inspection were as depected on the provided application maps, and n o other issues were <br />noted during the inspection. No objections have been received by DRMS during the public comment period.
