4. Certification: Kelley Trucking Inc.'s Annual Corporate Meeting minutes has been inserted
into the application package immediately following the certification page. This document
authorizes Cal Kelley to enter into contracts on behalf of Kelley Trucking Inc.

2. Maps of Affected Lands, General Requirements: The maps of the Affected Lands have been
brought into compliance with Rule 6.2.1 (2). Maps were prepared and stamped by a
registered Professional Land Surveyor.
3. Exhibit D: The Mining Plan Map has had the limits of the permit boundary updated so that
the haul road from Watkins Rd. to the mining operations will be within the limits of the
Affected Lands. The 21.8 acre area identified by Brock Bowles during the site inspection on
October 4, 2019 has been outlined on the Mining Plan map as well as the Pre-Mining Plan
Map as "Previously Affected Lands".The Updated Mining and Pre-Mining Plan Maps are
included in Exhibit C.
4. Exhibit R: A Fedex shipment receipt has been inserted to Exhibit R showing the
Incompleteness Response has been mailed to the Adams County Clerk and Recorder.
