Environmental Protection Facility Certification <br /> Climax Mine — Permit No. M-1977-493 <br /> Technical Revision 21, 29 <br /> Mayflower Flood Bypass Tunnel Extension (Phase II) <br /> The Mayflower Flood Bypass Tunnel was approved in Technical Revision No. 21 and No. 29 to <br /> Mine Permit No. M-1977-493. This project is located at Climax Molybdenum Company's Climax <br /> Mine in Summit County. The project includes the construction of a lined water conveyance tunnel <br /> used in part to prevent dam failure by allowing for emergency release of flood waters from the <br /> Mayflower Tailing Storage Facility. The Phase II project elements constructed in 2019 and 2020 <br /> have included an extension to the main tunnel, tunnel spur no. 2, two additional muck bays, <br /> stabilization measures and lining of the excavated rock surfaces, and a lined raise to the ground <br /> surface. The undersigned was responsible for the construction oversight for this project, including <br /> the inspection of materials used, survey control, and the field testing and measurements to ensure <br /> compliance with the project design drawings and technical specifications. <br /> The contractor has substantially completed the construction for this project and it has been <br /> determined to be functional. <br /> Pursuant to Rule 7.3.2, the Professional Engineer who has signed and sealed below certifies that <br /> the project was completed in general conformance with the approved design as submitted to the <br /> Colorado Division of Reclamation, Mining and Safety.
