PERMIT#:M-1978-014 <br /> INSPECTOR'S INITIALS:ECS <br /> INSPECTION DATE:February 5,2020 <br /> OBSERVATIONS <br /> This was a routine monitoring inspection of the North Chieftain Mine, a 126.4 acre 112c construction materials <br /> operation located at the NE corner of the intersection of Highway 470 and West Alameda Parkway. This <br /> inspection was conducted by Eric Scott of DRMS, who was joined by operator representative Jason McGraw <br /> for the inspection. The current bond posted for this site is $73,431. The site has not been actively mined since <br /> 2006, however material has continued to be utilized from existing stockpiles on site. Mr. McGraw stated that <br /> the mine is planning to resume mining activity in the summer of 2020. <br /> Stockpiles of topsoil, overburden, and product are present on the site and are well vegetated and stable (Photos <br /> 2, 3). The west, south and northwest boundaries of the permit area are defined by existing fences, and the site <br /> identification sign was present at the site access gate located off the north side of Alameda Parkway(Photo 1). <br /> Problems observed during the inspection have been identified at the beginning of the report and consist of the <br /> missing permit boundary markers on the east and northeast portions of the permit. These issues will need to be <br /> resolved by the corrective action dates listed above. It should also be noted that recreational trails associated <br /> with the Green Mountain Open Space Park now pass through the unaffected eastern portion of the permit area. <br /> Because the reclamation bond has not been adjusted in quite some time, the Division may also calculate a surety <br /> increase to account for increased costs to the state for the proposed earthwork and revegetation. A surety <br /> increase revision will be sent to the operator under separate cover and the operator will have 60 days from the <br /> date of the surety increase to provide the revised bond amount to the Division. <br /> No other issues were noted during this inspection. <br /> Inspection Contact Address <br /> Jason McGraw <br /> General Shale Brick,Inc. <br /> 1845 W.
