PERMIT#: M-2019-052 <br /> INSPECTOR'S INITIALS: ECS <br /> INSPECTION DATE:January 15,2020 <br /> OBSERVATIONS <br /> This was a pre-operational inspection for a proposed 206 acre 112c sand mine. The site is located in Weld <br /> County north of Keensburg CO one mile north of the intersection of CR59 and CR20 on the east side of CR59. <br /> This permit will overlap/include the current 10 acre 110c permit held by the Colorado Sand Company for the <br /> Keensburg Project (M2019027) which was also inspected during the course of this site visit. The inspection <br /> was conducted by Eric Scott of the Division of Reclamation Mining and Safety (DRMS) who was accompanied <br /> on-site by plant manager Mike Manke, and by permitting consultant Miles Blair of Applegate. Only two issues <br /> were observed during this inspection. The issues have been detailed below and will need to be corrected as <br /> directed by the summary on the first page of this report. <br /> The notice of permit applied for sign (Photo 1) was observed in 2 locations, one near the site entrance and one <br /> further north along CR59. However, the sign near the site entrance should be moved to where the site access <br /> road intersects with CR59, so it is visible from the access road. This is also true for the future Mine <br /> Identification sign for the site. <br /> Photo 2 shows the recently placed SW corner permit boundary marker. Although the majority of the new <br /> permit boundary markers were able to be located during the inspection, almost all had been knocked down by <br /> the cattle that are currently grazing within the proposed permit area, and will be difficult or impossible to <br /> locate in the near future. The permit boundary markers—especially corner markers—will need to be replaced <br /> with something that will not be knocked down by cattle or obscured in the existing vegetation (Photo 3). <br /> Markers should be placed at a frequency that will allow site workers to maintain visual contact between <br /> markers while working near the permit boundary. <br /> These issues should be corrected and proof submitted to the Division of the corrective actions taken by the <br /> corrective action due date shown on the first page of this report (Jan 31, 2020). <br /> No other items of concern were noted during this pre-operational inspection and conditions within and <br /> adjacent to the proposed permit were observed to match the information provided within the submitted <br /> application. <br /> Inspection Contact Address <br /> Mike Manke <br /> PAS Mining Solutions LLC <br /> 842 W Sam Houston Parkway N <br /> STE 110 <br /> Houston,TX 77024 <br /> CC: <br /> Seth Sanders <br /> Guidon Holdings LLC <br /> 842 W Sam Houston Parkway N <br /> STE 110 <br /> Houston,TX 77024 <br /> Page 2 of 4 <br />
