PERMIT#:M-2011-038 <br /> INSPECTOR'S INITIALS:SJM <br /> INSPECTION DATE: August 14,2019 <br /> OBSERVATIONS <br /> This was a routine inspection conducted by the Division as part of its monitoring of Construction Materials 111 <br /> permits. Mr. Steven Winer and Ms. Kathy Winer were present during the inspection. The site was not active <br /> on the day of the inspection. <br /> The required permit ID sign was not observed posted at the entrance to the permitted area. <br /> In October of 2018 the property was sold to a Mr. Bret Mouse, who was also on site during the inspection. At <br /> this time there is not legal right of entry, for the lease expired with the previous landowner in 2013. Mr. <br /> Mouse regraded the remaining stockpiles to grade. <br /> There was little or no noxious weeds on site. <br /> Based on the conditions observed during the inspection a request to release the entire permit area(s) is <br /> recommended. The Operator must submit a request that meets the process requirements of Rule 4.17. Rule <br /> 4.17 states the request must be sent via certified mail, include the address and phone number of all owners of <br /> property and such request shall state that the operator has achieved all requirements of the approved <br /> reclamation plan. <br /> If this request is submitted within 60 days of this inspection report, the August 14, 2019 inspection can be <br /> considered the release inspection. <br /> Photos that show the conditions observed during the inspection are included at the end of this report. <br /> Responses to this inspection report should be directed to Stephanie Mitchell at the Division of Reclamation, <br /> Mining and Safety, Grand Junction Field Office, 101 South 3rd Street, Room 301, Grand Junction, Colorado <br /> 81501, phone no. 303-866-3567 ext 8180. <br /> Page 2 of 4 <br />
