The following exhibits pertaining to this amendment were previously submitted with Amendment AM-3, approved April 14, 2004 and remain unchanged for this amendment unless altered as described below. Exhibit A- Legal Description remains unchanged from that found in Exhibit A of Amendment AM-3 approved April 14, 2004. Exhibit B—Index Map can be found in Exhibit B in Amendment AM-3 approved April 14, 2004. Exhibit C- Pre-mining and Mining Plan Maps can be found in Exhibit C in Amendment AM-3 approved April 14, 2004. Since mining was completed on this site, current conditions are depicted by the 2019 Annual Report Map. Exhibit D—Mining is complete on this property. There will not be any further mining. Exhibit E- Reclamation Plan can be found in Exhibit E in Amendment AM-3 approved April 14, 2004. This amendment changes the post-mining land use for this property from wildlife habitat to general agriculture. All mining and reclamation on this property has been completed according to the original reclamation plan in Amendment AM-3 approved April 14, 2004, except total re-vegetation of the site. This amendment removes re- vegetation from the reclamation plan. Exhibit F- Reclamation Plan Map can be found in Exhibit F in Amendment AM-3 approved April 14, 2004. The Annual Report Map for 2019 for this Permit is attached and shows the current status of reclamation for this site. Exhibit G -Water Information is the same as in Amendment AM-3 approved April 14, 2004. Exhibit H -Wildlife Information is the same as in Amendment AM-3 approved April 14, 2004. Exhibit I -Soils Information is the same as in Amendment AM-3 approved April 14, 2004. Exhibit J -Vegetation Information is the same as in Amendment AM-3 approved April 14, 2004. Exhibit K-Climate information is the same as in Amendment AM-3 approved April 14, 2004. Exhibit L- Reclamation Costs. All reclamation on this site has been completed as described in the Reclamation Plan in Exhibit E in Amendment AM-3 approved April 14, 2004. except re-vegetation of the site. If this amendment is approved,then the reclamation costs will be $0. Exhibit M—Other Permits and Licenses are the same as in Amendment AM-3 approved April 14, 2004. Exhibit N—Source of Legal Right to Enter can be found in the Succession of Operator application approved June 10, 2013. Exhibit 0—Owners of Record of Affected Land and Owners of Substance to be Mined can be found in the Succession of Operator application approved June 10, 2013. Exhibit P—Municipalities within Two Miles. This property is within the city limits of Delta, Colorado.
