(CSQ), through its consultant Greg Lewicki & Associates, <br /> PLLC (GLA), hereby submits this report concerning the above-referenced incident. This <br /> report is submitted pursuant to and in satisfaction of the requirements set forth in Rule <br /> 8.2.3 of the Mineral Rules and Regulations of the Colorado Mined Land Reclamation <br /> Board for Hard Rock, Metal and Designated Mining Operations and in the above-reference <br /> Inspection Report. <br /> The report is divided into eight principal parts. First, an overview of the events that <br /> occurred regarding the recent accidental release of diesel fuel at the mine is provided. <br /> Second, a brief description of the mine, and in particular those parts of the mine that were <br /> involved in the diesel fuels, is presented. Third, information is provided in regard to each <br /> of the four items/requests presented in Rule 8.2.3 and the Inspection Report. Fourth, a <br /> timeline is presented in great detail as requested by the Division. The fifth and sixth parts <br /> of this report discuss factors that contributed to the spill. Finally, the seventh and eighth <br /> sections detail the mitigation efforts and ongoing mitigation plans and procedures. Much, <br /> if not all, of this information has been shared with the Division as a result of discussions <br /> between GLA and Division representatives, and visits to the mine sites by the Division. <br /> 1 <br />
