I've responded to the DRMS on <br /> several of the points mentioned. One of the requirements was to provide Park County with the <br /> DRMS's Notice along with my response. My understanding is that this document should be <br /> attached to the public review copy of the original application held at your offices. <br /> Per the DRMS Notice of Filing Deficiencies, dated October 2,2019 (attached),I have made the <br /> following corrections to the referenced Limited Impact Operations(110(l))Reclamation Permit <br /> Application Form which I submitted to Park County back in September 2019. <br /> • Pagel: <br /> o Add a File Number(M-2019-050)as assigned by the DRMS. <br /> o Question 6: Changed the surface owner from"Robert&Lauren Berry"to the <br /> "U.S. Forest Service". <br /> o Question 8: Changed the type of mining operation from"Placer"to"Surface". <br /> o I've attached a copy of only the edited Page 1 of my referenced application. <br /> • Exhibit G: <br /> o Attached is a copy of the U.S. Forest Service approval of the NOI and my <br /> referenced application. <br /> o I emailed the DRMS on October 14,2019 and provided a copy of the BLM <br /> Mining Claim for Robert Michael#1, CMC287911,and a BLM Report which <br /> shows both Robert and Lauren Berry as the claim owners and attached a copy <br /> hereto. <br /> Page 1 of 2 <br />
