Photos 1 and 2 show <br /> the area where the spill occurred before and after cleanup efforts were completed. <br /> The following points address the requirements of Rule 8, section 8.2.3: <br /> a. Actions taken to respond to and correct the emergency situation or condition <br /> • Around 2:30 pm on November 14, 2019 a power bump caused a <br /> communication error, resulting in a backflow of solution, overtopping a <br /> tank interior to the Mill, spilling approximately 200 gallons of process <br /> solution out of an overhead doorway. All solution remained on the mill <br /> liner. <br /> • Once identified, CC&V personnel quickly addressed the problem and <br /> returned flow to normal operation. <br /> • The impacted material was over-excavated and placed on the mill feed <br /> pile. <br />
