PERMIT#:P-2015-007 <br /> INSPECTOR'S INITIALS:ECS <br /> INSPECTION DATE:November 15,2019 <br /> OBSERVATIONS <br /> This inspection was conducted by Eric Scott of the Division of Reclamation Mining and Safety (DRMS) and the <br /> operator/permittee Thomas Miller. The site is located approximately 8 miles southwest of Deckers, CO in the <br /> Pike-San Isabel National Forest, and within the Hayman Fire burn scar. USFS was not present for this <br /> inspection. DRMS contacted USFS prior to the inspection to notify them of the release request and seek USFS <br /> concurrence, which was provided, and has been included in the site file. <br /> The operator had previously requested release of the site in 2017. It was determined at that time that <br /> although the earthwork was satisfactory, the vegetation was not yet sufficient to approve a release. The <br /> operator implemented a weed control plan, and continued to monitor site revegetation. <br /> DRMS received the operator's current release request on October 29, 2019, and subsequently notified the <br /> USFS of the release request on October 31, 2019. USFS had conducted an inspection of the site on August 23, <br /> _2019, an-' provided DRIVIS with d LUPY of the USFinspection report which had determined that the site <br /> reclamation was adequate at that time to support the full release of the site. During the DRMS inspection the <br /> site appeared stable and almost indistinguishable from the surrounding area. <br /> Photos of the reclaimed area have been attached to this report. Having received concurrence from USFS that <br /> the site has been adequately reclaimed, DRMS will approve the release of this NOI at this time. <br /> Inspection Contact Address <br /> Thomas G.
