Compliance With Mine Plan NA - Support Facilities Not On-site <br /> N -Other NA - Special Categories Of Mining <br /> NA -Processing Waste Y -Topsoil <br /> COMMENTS <br /> This was a complete inspection of the Seneca IIW Mine, C 1982057, operated by Seneca Property LLC. Mine <br /> representative Miranda Kawcak,was present throughout the inspection. The weather was cloudy with the <br /> temperatures near 50 degrees F. The main haul road provided access to the entire site. No offsite air or water <br /> quality issues were noted at the time of inspection. <br /> UPCOMING PERMITTING ACTIONS: <br /> Partial Phase I, II, and III Bond Release <br /> OUTSTANDING MAINTENANCE ITEMS: <br /> 1. One of the monitoring wells is been broken off. The operator agreed to repair this and,repressurize the <br /> others as needed. <br /> 2. Pressurize wells that have lost pressure. <br /> 3. Main haul road requires grading in selected steep areas due to rilling. <br /> 4. Culvert above pond 16 requires grass to be removed from the outlet end. <br /> 5. Erosional feature above culvert. <br /> 6. Exposed fabric and soil at top of dam at stock pond. <br /> INSPECTION: <br /> GENERAL MINE PLAN COMPLIANCE: <br /> It appeared that permitted activities were progressing as planned. Reclamation has, for the most part,been <br /> completed,with the exception of some small buildings that require demolition. Vegetation appeared well <br /> established in most places throughout the site. Constructed drainages appear to be well armored and functioning <br /> adequately. <br /> SIGNS and MARKERS—Rule 4.02 <br /> o Entrance sign and permit boundary markers were posted as required. <br /> o The mine site identification signs observed at the mine access points were intact, displaying all required <br /> information: mine name, operator name, address,phone number, DBMS permit number and contact information. <br /> Number of Partial Inspection this Fiscal Year: 2 <br /> Number of Complete Inspections this Fiscal Year: 2 <br /> Page 2 of 5 <br />
