May 03, 2018 <br /> Project ID: CCV_4059-3 <br /> ACZ Project ID: L43420 <br /> ACZ Laboratories, Inc. (ACZ) received 1 soil sample from HCltasca Denver, Inc. on March 29, 2018. The sample was <br /> received in good condition. Upon receipt,the sample custodian removed the sample from the cooler, inspected the <br /> contents, and logged the sample into ACZ's computerized Laboratory Information Management System (LIMS). The sample <br /> was assigned ACZ LIMS project number L43420. The custodian verified the sample information entered into the computer <br /> against the chain of custody(COC)forms and sample bottle labels. <br /> ;Holding <br /> All analyses were performed within EPA recommended holding times. <br /> :Sample Analysis <br /> This sample was analyzed for inorganic parameters. The individual methods are referenced on both the ACZ invoice and <br /> the analytical reports. The extended qualifier reports may contain footnotes qualifying specific elements due to QC failures. <br /> In addition the following has been noted with this specific project: <br /> The Sequential NAG results have been qualified with the N1 flag on the extended qualifier report. The chemist noted that <br /> only the first stage of the NAG workgroup was uploaded/reported in ACZ's system. The NAG benchsheet with subsequent <br /> stages was sent to the client via email. <br /> Some of the MWMT results have been qualified with the B1 flag on the extended qualifier report. The chemist noted that <br /> these analytes were detected in the method prep blank sample(PBS)above the method reporting limit. The MWMT fluid <br /> passes through the analytical column as part of the preparation procedure, resulting in trace contamination of some analytes. <br /> Lel�W-11 MM 418 Page 2 of 47 <br />
