I acknowledge that I have received,read and understand the Edward Jones Account Agreement("Account Agreement") and agree to its terms,and have received the Schedule of Fees,Margin Disclosure Statement and Statement of Credit Terms,Privacy Notice,and Revenue Sharing Disclosure. 2, 1 agree that Edward Jones is acting as agent and not in a fiduciary relationship with the trust. 3. 1,jointly and severalty with any co-trustees,agree that all of the Information contained in this document is accurate,and that the trust has not been revoked,modified,or amended in any manner that would cause the representations contained herein to be incorrect. I further agree that this Information shall remain in full force and effect until such time as Edward Jones Is notified in writing of any change in the information or authority reflected in this form.I,jointly and severally with any co- trustees and on behalf of the trust,agree for myself,my heirs,successors and assigns to fully release,indemnify,defend and hold harmless Edward Jones and its affiliates,principals,employees,agents,successors and assigns("Edward Jones') from and against any damages,losses,claims,liability actions and expenses(including attorney fees)resulting from or arising out of Edward Jones'reliance on the Information provided In this form. 4. 1 understand that,If a trustee marks the box below electing not to be authorized on this account,such trustee agrees that he or she will not be authorized to give instructions or request information related to this account, unless and until Edward Jones receives a new form signed by all current trustees changing such authorization. 5. 1 acknowledge that all taxable events for the current calendar year relating to this trust account shalt be reported by Edward Jones under the SSN/TIN provided above, regardless of when during the current calendar year the taxable event occurred.
