Scott August 7,2019 13:30 <br /> OPERATOR: OPERATOR REPRESENTATIVE: TYPE OF OPERATION: <br /> Yuma County Don Marr I I2c-Construction Regular Operation <br /> REF: Two problems were observed during this inspection.The most pressing issue is the presence on-site of <br /> stained soils due to leaks from heavy equipment. Although no equipment was present on site during the inspection, <br /> stained soils were noted in several locations in the area immediately north of the material stockpile(photos 4 and <br /> 5). This has been cited as problem#1 requiring the Operator to remove the stained soils,document the process and <br /> provide evidence of proper removal and disposal of stained soils from the site. <br /> The second issue is the current condition of the material stockpile located in the active area of the permit. Material <br /> has been removed from this pile in such a way as to leave a vertical slope approx. 20-30 feet high. This is an <br /> unstable configuration and could pose a safety hazard to equipment operators and on-site personnel if one of the <br /> vertical walls were to collapse. The material stockpile should be regraded to a more stable and safe configuration as <br /> soon as practically possible. <br /> Problem#1 is Being addressed by the land owner. Who will submit a report with all required documentation. <br /> Problem#2 has been addressed see attached photos. We have rounded off the stock pile using a dozer to eliminate <br /> any high walls.
