Construction associated with this certification report was <br /> completed in July 2019. <br /> The majority of the earthworks construction to the extent of the finished subgrade elevation <br /> below the SLF was performed concurrently with their Squaw Gulch VLF Phase 1 work prior to <br /> 2019.After January 2019,the Squaw Gulch VLF Phase 2A Part 1 area was constructed placing SLF, <br /> overlain with an 80 mil Linear Low Density Polyethylene(LLDPE)double sided micro-spike(DSMS) <br /> geomembrane, and covered with Drain Cover Fill (DCF). Construction within this area was <br /> completed on July 15, 2019. The Squaw Gulch VLF Phase 2A grading plan and typical VLF cross <br /> sections are shown on Drawings A40 and A60/A62 in the Issued for Construction (IFC) drawing <br /> set, respectively. <br /> 1.2. Parties Involved <br /> Work performed during the Squaw Gulch VLF Phase 2A Part 1 project was completed by several <br /> parties. Responsible parties involved in the project are listed below: <br /> Project management was provided by CC&V. <br /> • During the 2019 construction activities, Messrs. Steve Blaskovich, Jeff Gaul, and John <br /> Shiflett represented CC&V as the Construction Manager and Construction <br /> Superintendents, respectively. <br /> • During the 2016 construction activities, Messrs. John Cora, Ron Roberts, Ron DiDonato <br /> and Jeff Gaul represented CC&V as the Construction Manager and Construction <br /> Superintendents, respectively. <br /> Ames Construction Inc. (Ames)was contracted by CC&V as the general contractor responsible for <br /> all construction activities between January 2013 and January 2019. <br /> Tezak Heavy Equipment Co., Inc. (Tezak) was contracted by CC&V as the general contractor <br /> responsible for all construction activities after January 2019 for the Phase 2A project. <br /> Edward-James Surveying, Inc. (EJS) performed surveying for all construction activities performed <br /> by Tezak. <br /> Tetra Tech,formerly known as American Environmental Group, Ltd. (AEG), was subcontracted by <br /> Tezak to perform the Phase 2A geomembrane installation. <br /> NewFields provided field engineering and construction QA/QC testing and inspection starting on <br /> March 1, 2016. A Staff Schedule of NewFields Personnel is presented in Appendix A. <br /> AMEC provided field engineering and construction QA/QC testing and inspection from the start <br /> of Phase 1 construction until February 29, 2016. Staff Schedules of AMEC Personnel are <br /> presented in previous ROC reports. <br /> Foresight West Surveying, Inc.
