PERMIT#: M-1984-041 <br /> INSPECTOR'S INITIALS: SJM <br /> INSPECTION DATE: July 8,2019 <br /> OBSERVATIONS <br /> This inspection was conducted as part of the Division of Reclamation, Mining and Safety's (Division) normal <br /> monitoring program. Jon Edeen representing the Operator, was present during this inspection. Jessica Lopez- <br /> Pearce representing the BLM was also present. Photographs are included at the end of this report in order to <br /> illustrate some of the conditions observed during this inspection. <br /> The Eagle Gypsum Mine is located approximately 2 miles north of Gypsum in Eagle County Colorado and <br /> accessed from the Trail Gulch Road. The site is a 112c operation that includes a total of 830 permitted acres. <br /> The Division currently holds a financial warranty amount of$1,887,480.38 for this site. The approved post <br /> mining land use is Wildlife Habitat. The Division will review the reclamation costs for this site in order to <br /> ensure the financial warranty reflects the actual current cost of fulfilling the requirements of the Reclamation <br /> Plan. The Operator will be notified if the Division determines that the current financial warranty amount is <br /> insufficient. <br /> Adequate mine identification signage was noted at the entrance to the site. The affected area was adequately <br /> marked with fencing and posts. The ore stockpile is currently at 27,000 tons. <br /> Ponds and Stormwater structures were inspected. Pond 1 appears to be functioning properly. Pond 1 b has a <br /> large gully forming at the entrance to the pond. This is considered a problem at the beginning of the <br /> inspection report. The Division recommends rip rap be added to the gully and some check dams. Pond 1 a <br /> needs repair. The liner is showing through the rip rap channel. <br /> Stormwater Structure, 2,3,4,5 and 6 were all inspected and appear to be working properly. The Operator is <br /> encouraged to keep up maintenance and clean out the features. <br /> A fence above the highwall was installed in 2016 and was inspected. <br /> Fuel storage is with adequate secondary containment. <br /> Responses to this inspection report should be directed to Stephanie Mitchell at the Division of Reclamation, <br /> Mining and Safety, Grand Junction Field Office, 101 South 3rd Street, Room 301, Grand Junction, Colorado <br /> 81501, phone number (303)866-3567. <br /> Page 2 of 5 <br />
