Compliance With Mine Plan NA - Support Facilities Not On-site <br /> N -Other NA - Special Categories Of Mining <br /> N -Processing Waste Y -Topsoil <br /> COMMENTS <br /> This was a complete inspection of the Hayden Gulch Loadout; DRMS Permit No. C-1992-081, operated by <br /> Hayden Gulch Terminal, LLC. This inspection was conducted by Tabetha Lynch with the Colorado Division of <br /> Reclamation, Mining, and Safety(Division). The weather was clear and warm during the inspection. Records <br /> were checked at the Foidel Creek Mine office on 7/23/2019 and the site inspection was conducted on the afternoon <br /> of 7/24/2019. No cattle were present at the time of inspection <br /> AVAILABILITY OF RECORDS—Rule 5.02.4(1): <br /> See the DRMS Availability of Records at the end of this report <br /> HYDROLOGIC BALANCE -Rule 4.05 <br /> Drainage Control 4.05.1,4.05.2,4.05.3; Siltation Structures 4.05.5,4.05.6; Discharge Structures 4.05.7, 4.05.10; <br /> Diversions 4.05.4; Effluent Limits 4.05.2; Ground Water Monitoring 4.05.13; Surface Water Monitoring 4.05.13; <br /> Drainage—Acid and Toxic Materials 4.05.8; Impoundments 4.05.6, 4.05.9; Stream Buffer Zones 4.05.18: <br /> The two ponds on site were holding some water but not discharging. Spillways were clear and functional. <br /> Monitoring well within the permit boundary was observed and appeared functional and competent. All culverts <br /> were clear and functional. No erosional features were noted. <br /> GENERAL MINE PLAN COMPLIANCE: <br /> The parking lot constructed just south of Hwy 40 is currently covered in weeds. The topsoil pile <br /> associated with this parking lot is also covered in weeds. If this topsoil pile is to stay in it's current location <br /> additional protective measures will be needed and it will need to be seeded with a temporary mix. Parking lot will <br /> need to weed mitigation if it is to remain as a gravel pad, <br /> SIGNS AND MARKERS—Rule 4.02: <br /> Permit boundary markers were present and included the required information. Stream buffers sign was <br /> present.
