PERMIT#: M-2014-070 <br /> INSPECTOR'S INITIALS:ECS <br /> INSPECTION DATE: July 24,2019 <br /> OBSERVATIONS <br /> This was a routine monitoring inspection of the permit for the 161 acre 112c GoodMiller#2 Pit, M2014-070. <br /> The inspection was conducted by Eric Scott of DRMS and permittee representative/operator Mike Hamilton. <br /> The site is located in Weld County approximately one mile north of the intersection of county road 80 and <br /> county road 115 on the west side of county road 115. The site is also located immediately west of the Weld <br /> County 112c Ashbaugh Pit. <br /> The corners of the permit were marked at the time of the inspection with lath and re-bar pins on the south <br /> and west boundaries, and property fence lines on the north and east boundaries. The disturbed area was <br /> within an approximately 75 acre fenced area within the permit. As long as the disturbance remains within the <br /> currently fenced area there should be no boundary issues. <br /> No mining is currently occurring at the site, and some earthwork is underway to reclaim portions of the <br /> disturbed area. The majority of the area within the 75 acre fenced portion of the permit has been affected <br /> either by the pit excavation itself, stockpiles of overburden and topsoil, or access roadways. The currently <br /> approved permit imposes the following limitations on the site disturbance: <br /> Maximum length of 700 feet of slope that requires backfilling to 3H:1V <br /> Maximum of 28 acres that will require topsoil replacement at any one time <br /> Maximum of 30 acres that will require seeding/mulching/fertilizing at any one time. <br /> During this inspection it was observed that all of these limits have been exceeded at this time. This is cited <br /> on the first page of this report as a problem for failure to follow the approved mining plan. The operator <br /> will be required to conduct reclamation at the site to bring the disturbed areas to within the approved <br /> allowable limits as stated above, or submit a technical revision to the mining plan to modify the disturbance <br /> limits and increase the associated bond by the corrective action date shown (September 30, 2019). <br /> The crushed culverts located at the NW corner of the permit should also be removed from the site and <br /> properly disposed of(photo 4). <br /> No other items/concerns were noted during this inspection, and the attached photos generally depict the <br /> condition of the permit area. <br /> Inspection Contact Address <br /> Mike Miller <br /> VESCO Consulting Services, LLC <br /> 1624 E Hwy 34 <br /> Greeley, CO 80633 <br /> Page 2 of 4 <br />
