PERMIT#:M-2002-120 <br /> INSPECTOR'S INITIALS: ECS <br /> INSPECTION DATE: July 10,2019 <br /> OBSERVATIONS <br /> This was a pre-operation monitoring inspection conducted for AM01 by DRMS at the Hall Irwin Bernhardt <br /> Resource site. There are 286.14 acres currently permitted at this 112c site. Due to routing of the conveyor to <br /> Cell 4 across unpermitted property, the operator was required by the MLRB to submit AM01 to incorporate <br /> that area into the permit. AM01 will increase the permit area to approx. 292 acres by incorporating the 6 acre <br /> parcel immediately north of the City property into the permit area. <br /> The inspection was conducted by Eric Scott of DRMS. The DRMS representative traveled to the site and met <br /> permittee representative Mark Johnson of Bestway Concrete at approximately 1145. Although this inspection <br /> focused primarily on the 6 acre parcel to be amended into the permit, it was noted that the banks of Cell 3 <br /> have been fully re-sloped to 3h:1v, and the cell was being filled by Central Water Conservancy District during <br /> the site visit (photos 1-2). Central Water has also installed a rock lined inlet/spillway in the NW corner of Cell <br /> 3 as seen in photo 1. <br /> During this inspection it was noted that the permit boundary for the amended parcel had not been fully/ <br /> appropriately marked. It was also noted that the existing fence in the SW corner of the amended parcel does <br /> not correspond to the permit/parcel boundary as shown on the maps in AM01. Prior to the approval of AM01 <br /> the permittee will be required to appropriately mark the permit boundary for the amended area, and submit <br /> proof to DRMS that this has been completed. If it is determined that the permit boundary should follow the <br /> existing fence in the SW corner of the amendment area, the permit boundary maps and acreage totals <br /> included in AM01 will need to be updated appropriately to show this modification. <br /> No other issues were noted during this inspection. <br /> Inspection Contact Address <br /> Mark Johnson <br /> Bestway Concrete Company <br /> 301 Centennial Dr. <br /> Milliken,CO 80543 <br /> Page 2 of 3 <br />
