PERMIT#:P-2015-014 <br /> INSPECTOR'S INITIALS: DMC <br /> INSPECTION DATE:August 8,2018 <br /> OBSERVATIONS <br /> This inspection was conducted as part of the Division of Reclamation, Mining and Safety's (Division) normal <br /> monitoring program. A representative for the operator was not present. Photographs are included with this report <br /> in order to illustrate the conditions observed. <br /> The Halfmoon Site is located approximately five miles southwest of Leadville and accessed from CR I IA. This <br /> is an exploration operation that was authorized to drill three boreholes. According to the operator's abandonment <br /> report the holes were drilled and reclaimed in June 2015. Pursuant to Rule 4.1.2 (2), a financial warranty is not <br /> required for this site. <br /> This inspection confirmed that there is no unreclaimed disturbance at the site.
