COLORADO OPERATIONS <br /> ® Climax Molybdenum Henderson Mill <br /> A Freeport-McMoRan Company 19302 County Road 3 <br /> Parshall, CO 80468 <br /> Phone(303)569-3221 <br /> June 14, 2019 REC �" <br /> Via Email and UPS: 1Z 804 641 01 9533 0564 JUN 17 2019 <br /> Mr. Peter Hays <br /> DIVISION OF RECLAMATON Division of Reclamation, Mining and Safety MININGANpI;f►¢E7'y <br /> 1313 Sherman St., Rm. 215 <br /> Denver, CO 80203 <br /> Re: Follow-up Report for Fire Water Line Incident, Permit No. M-1977-342 <br /> Dear Mr. Hays: <br /> Henderson Mill is providing this written follow-up report for an incident which you were notified about by <br /> phone on October 29th and by follow-up notice on November 2"d <br /> As mentioned in the notification, a pool of water was identified underneath the Reclaim #1 conveyor and <br /> subsequent tests revealed there was a leak in the fire water distribution system at this location (Image#1). <br /> At the time this section of the fire water distribution system was isolated for the winter until further <br /> investigation could be completed in the spring. <br /> Excavation of the area commenced on May 1st and a leak was discovered in the pipeline approximately 15 <br /> feet west of Fire Hydrant#7 (Images#2 ). The hole was repaired with a threaded plug and welded in <br /> place (Image #4). The pipeline was then taken out of isolation and pressurized to check for leaks and <br /> confirm the repair was effective. No other leaks were identified and the welded plug was successful. <br /> Cathodic protection was added to the exposed section of the pipeline and the excavation was backfilled <br /> with crushed rock and gravel for drainage as well as sand.
