RE: Von Doose Mine,Permit M-2019-031,Address Correction,Re-Notification,and Signed
Affidavit for On-Site Notice for 110(1)Application
Dear Mr. Lucas,
During internal review of the above referenced application, it was discovered that the address for Atlas
Mining and Reclamation,LLC(AMR)was incorrect on Page 2 of the 110(l)Application Form and on the
On-Site Notice posted at the proposed entrance to the Von Doose Mine. Additionally, AMR did not
previously include a signed affidavit that the On-Site Notice was posted according to Rule 1.6.2(1)(b).
To correct these errors AMR has taken the following actions:
1. Mailed new notifications to both the Ouray County Board of County Commissioners(BOCC)and
the Shavano Soil Conservation District(Shavano). Enclosed with the new notifications were the
respective notification templates provided by the Division for each entity and a complete
replacement copy of the l I0(l)Application Form with the corrected address.
2. Replaced the On-Site Notice at the proposed entrance to the Von Doose Mine showing AMR's
correct address and telephone number.
3. Signed an affidavit attesting to the proper posting of the On-Site Notice.
4. Corrected the errors and omissions in the copy of the application held by the Ouray County Clerk
(Clerk)for public review.
5. Obtained a signed statement from the Clerk that the public copy has been updated with the
corrections.
For the Division's files regarding all of the above,please find enclosed with this letter the following:
1. A replacement 110(l)Application Form with the corrected address on page 2.
2. Two copies of a replacement EXHIBIT J containing(1)the certified mailing receipts for the new
notifications to the BOCC and Shavano; (2)a current photograph of the On-Site Notice; and(3)a
signed affidavit that the On-Site Notice was posted according to Rule 1.6.2(1)(b).
3. Two copies of an addendum to EXHIBIT I containing the signed statement from the Clerk.
