NEWMONT
GOLDCORR
inch steel header and the 12 inch HDPE pipe. Upon reaching pressure
equalization between the two pipes, the structural capacity of the 12 inch
HDPE pipe was exceeded and cracked and then broke at the factory weld.
A formal investigation into the event will be completed.
b. Any known or adverse impacts to human health, property or the environment
• No known impacts to human health, property or the environment were
observed. Leach pad staff responded quickly, turned off valves, cleaned-
up off liner cyanide affected soil, and applied calcium hypochlorite to
mitigate any remaining cyanide.
c. Monitoring and analyses that are necessary to evaluate the situation and
corrective actions
• Sampling occurred pre-cleanup of the off-liner impacted soil and post
clean-up. Analyses will be available in the next 30-45 days.
e. Results of the Operator's investigation
After initial review of the incident, it appears this release was due to a valve being
closed at the 9,750 elevation on the 12 inch HDPE pipe which caused a pressure
differential between the 30 inch steel header and the 12 inch HDPE pipe. Upon
reaching pressure equalization between the two pipes, the structural capacity of the 12
inch HDPE pipe was exceeded and cracked and then broke at the factory weld.
