the ten years of monitoring following installation, there was no target constituents found to be above the <br /> point of compliance levels for this well. BMRI currently believes that the integrity of well M-15 has <br /> become suspect or compromised due to a partial collapse of this well. Due to a restriction in access in the <br /> well, BMRI is no longer able to sample this well. Since the integrity of this well is compromised and the <br /> former milling operations buildings no longer exist and this area reclaimed, BMRI requests to <br /> decommission well M-15 and remove it as a point of compliance as it is no longer needed by other <br /> programs. <br /> Well M-15 Decommissioning <br /> The method for decommissioning will be determined based upon well construction, condition at time of <br /> inspection, constituent levels and environmental parameters. BMRI proposes to decommission well M- <br /> 15 according to the attached procedure and will utilize a licensed driller in the State of Colorado for this <br /> work. The specific method of decommissioning will be determined according to the procedure and field <br /> conditions found at the time of inspection. The method selected will be designed to protect groundwater <br /> and will be implemented according to current best engineering practices while following all applicable <br /> federal, state and local regulations. <br /> If you are in agreement with this Technical Revision, please notify us as soon as possible so that BMRI <br /> can proceed with contracting and initiating field work for completion this summer.
