Photos 1 and 2 show the area where the <br /> spill occurred. <br /> The following points address the requirements of Rule 8, section 8.2.3: <br /> a. Actions taken to respond to and correct the emergency situation or condition <br /> • Around 4:30 pm on May 8, 2019 the barren solution pumps at the ADR2 <br /> malfunctioned, causing the barren solution tank to fill faster than the remaining <br /> pumps could pull solution out of the tank and up to the leach pad. <br /> • Approximately 2,000 gallons of barren solution spilled on the North side of ADR2. <br /> The spill occurred over the triple-lined Pregnant Solution Storage Area (PSSA). <br /> All solution was contained and remained on liner. <br /> • CC&V personnel quickly addressed the problem with the pumps to balance flow <br /> across the plant. <br /> • This spill was due to a miscommunication between automated pumps and a <br /> formal investigation into the event will be completed. <br /> b. Any known or adverse impacts to human health, property or the environment <br />
