Hwy. 160,#2 <br /> Pagosa Springs, Colorado 81147 <br /> Dear Mr. Brown: <br /> We are responding to your request for a Department of the Army permit for the Bootjack <br /> Ranch Fish Habitat Improvement project. This project involves activities in waters of the United <br /> States to install numerous in stream structures enhance fish habitat. The project is located on the <br /> West Fork of the San Juan River at 12500 State Highway 160,within Section 32 of Township 37 <br /> North, Range 2 West,New Mexico Principal Meridian, Mineral County,Colorado. <br /> Based on the information you provided,the proposed activity in approximately 2.86 miles of <br /> stream channel enhancement is authorized by Regional General Permit number 12. Your work <br /> must comply with the general terns and conditions listed on the enclosed Regional General Permit <br /> information sheets and the following conditions: <br /> Special Conditions <br /> l. In order to provide a permanent record of the completed work, you shall provide a <br /> complete set of as-built drawings within 90 days following the completion of work. <br /> Drawings shall contain enough detail to compare post-construction conditions to <br /> baseline conditions. You shall monitor all completed work in accordance with the <br /> stated project goals and objectives(e.g.,fish populations, spawning gravel, <br /> invertebrate populations,etc.). You must submit a monitoring report for evaluation <br /> of objectives and effectiveness by December 31 of the third year after project <br /> completion. <br /> 2. You must sign the enclosed Compliance Certification and return it to this office within <br /> 30 days after completion of the authorized work. <br /> This verification is valid for two years from the date of this letter or until the Regional General <br /> Permit is modified,reissued,or revoked,whichever comes first. Failure to comply with the General <br /> Conditions of this Regional General Permit,or the project-specific Spacial Conditions of this <br /> authorization,may result in the suspension or revocation of your authorization. <br /> 19 <br /> I I I 03/28/2019 i <br />
