PART II <br /> Permit No.: COR400000 <br /> ii. (ii) a senior executive officer having responsibility for the overall operations of a <br /> principal geographic unit of the agency. (e.g., Regional Administrator of EPA) <br /> 2. Electronic Signatures <br /> For persons signing applications for coverage under this permit electronically, in <br /> addition to meeting other applicable requirements stated above, such signatures must <br /> meet the same signature, authentication, and identity-proofing standards set forth at <br /> 40 CFR S 3.2000(b) for electronic reports (including robust second-factor <br /> authentication). Compliance with this requirement can be achieved by submitting the <br /> application using the Colorado Environmental Online Service (CEOS) system. <br /> 3. Change in Authorization to Sign <br /> If an authorization is no longer accurate because a different individual or position has <br /> responsibility for the overall operation of the facility, a new authorization must be <br /> submitted to the division, prior to the re-authorization, or together with any reports, <br /> information, or applications to be signed by an authorized representative. <br /> L. REPORTING REQUIREMENTS <br /> 1. Planned Changes <br /> The permittee shall give advance notice to the division, in writing, of any planned <br /> physical alterations or additions to the permitted facility in accordance with 40 CFR <br /> 122.41(1) and Regulation 61.8(5)(a). Notice is required only when: <br /> a. The alteration or addition to a permitted facility may meet one of the criteria for <br /> determining whether a facility is a new source in 40 CFR 122.29(b); or <br /> b. The alteration or addition could significantly change the nature or increase the <br /> quantity of pollutants discharged. This notification applies to pollutants which are <br /> subject neither to effluent limitations in the permit, nor to notification requirements <br /> under 40 CFR 122.41(a)(1). <br /> 2. Anticipated Non-Compliance <br /> The permittee shall give advance notice to the division, in writing, of any planned <br /> changes in the permitted facility or activity that may result in noncompliance with <br /> permit requirements. The timing of notification requirements differs based on the type <br /> of non-compliance as described in subparagraphs 5, 6, 7, and 8 below. <br /> 3. Transfer of Ownership or Control <br /> The permittee shall notify the division, in writing, ten (10) calendar days in advance of a <br /> proposed transfer of the permit. This permit is not transferable to any person except <br /> after notice is given to the division. <br /> Page 25 of 33 <br />
