PERMIT #: M-1995-060 <br />INSPECTOR'S INITIALS: SJM <br />INSPECTION DATE: March 28, 2019 <br />OBSERVATIONS <br />This was a monitoring inspection of the Butcher Pit; M-1995-060 operated by Southway Construction <br />Company, Inc.. This site is located 1/2 mile southwest of Alamosa, Colorado in Alamosa County. I, Stephanie <br />Mitchell of the Colorado Division of Reclamation, Mining and Safety conducted the inspection. Mr. Ralph <br />Martinez of Southway Construction Company, Inc. accompanied me on the inspection. <br />This is a 40 acre 112c mining operation for sand and gravel with a maximum allowed disturbance of 29 acres. <br />The current disturbed acreage is appears to be right at 9 acres. The Division currently holds a financial <br />warranty amount of $61,370 for the site. The Division will review the reclamation costs to determine of the <br />financial warranty is adequate. If it is not, the Division will inform the Operator of an updated cost estimation <br />via a separate cover. <br />A mine identification sign was in place at the entrance to the mine site. All permit boundaries were all clearly <br />marked delineating the permit area. There is also some illegal dumping at the site. The Division encourages <br />to take extra measure to ensure it does not become a problem, such as fencing, signage, and road blocks. <br />The site has been inactive for over 2 years. The Division is requiring the Operator to request Temporary <br />Cessation status. This is noted as a problem at the beginning of this inspection report. <br />There was some water retained for over 72 hours, but it is a result of spring thaw and is expected to be dry <br />again after the snow melts. <br />There is no equipment or fuel stored on site. <br />Responses to this inspection report should be directed to Stephanie Mitchell at the Division of Reclamation, <br />Mining & Safety, Grand Junction Field Office, 101 South 3rd Street, Room 301, Grand Junction, Colorado <br />81501, phone number 970.242.5025. <br />Page 2 of 5 <br />
