PERMIT #: M-1992-060 <br />INSPECTOR'S INITIALS: SJM <br />INSPECTION DATE: March 27, 2019 <br />OBSERVATIONS <br />This was a normal monitoring inspection of the Lagoon Pit, DRMS File No. M-1992-060 operated by the City of <br />Monte Vista. This site is located northeast of Monte Vista, Colorado in Rio Grande County. I, Stephanie <br />Mitchell of the Colorado Division of Reclamation, Mining and Safety conducted the inspection. Mr. Lowell <br />Bolts and Tony Martinez of the City of Monte Vista accompanied me on the inspection. <br />This is a 6 acre 110c operation. Mining is complete and the Operator has completed reclaiming the site. The <br />perimeter side slopes of the pit have been graded to a 4:1 horizontal to vertical ratio. Yellow Sweet Clover, <br />Western Wheatgrass, Timothy and Willows have begun to establish at the site. The post mining land use is <br />Rangeland. <br />The Lagoon Pit is ready for release of the performance warranties. The Division does not hold a Financial <br />Warranty on the site due to the Operator being a Government Entity. The Division suggests that the City Of <br />Monte Vista request a release of this site within the next 30 days. According to Rule 4.17.1, the Operator may <br />file a written request for release of reclamation responsibility to the Division. The Operator shall include in the <br />notice to the Division the names and address and phone numbers of all owners of record to the affected land. <br />The written notice requesting said release shall be sent by certified mail. The notice shall contain a signed <br />statement by the Operator that all applicable portions of the reclamation plan requirements have been <br />satisfied in accordance with the Rules and applicable requirements under the Act. Please refer to the Divisions <br />Rules and Act, specifically Rule 4.17.1 for specific steps to take regarding release of performance and financial <br />warranties. <br />Noxious weeds appeared to be under control and visibly dead.
