PERMIT#:M-1974-086 <br /> INSPECTOR'S INITIALS: ECS <br /> INSPECTION DATE:February 12,2019 <br /> OBSERVATIONS <br /> This was a pre-operational inspection conducted by Eric Scott of the Division of Reclamation Mining and Safety(DRMS) <br /> for the Ralston Quarry AM04. The Ralston Quarry is a 112c permit operated by Asphalt Paving Company(APC) in <br /> Jefferson County approximately 2.25 miles north of Golden and 0.6 miles east of Hwy 93. AM04 is a MLRB-ordered <br /> amendment submitted to add acreage to incorporate into the permit area land that had been affected by a landslide in <br /> June of 2018. The operator has also added additional stockpile area to the SW area of the previous permit. The <br /> required notice signs were present at the entrance to the site from Highway 93 (photo 1). <br /> Eric Scott travelled to the site and met with Mark Martin,the site health and safety officer. Also present for this <br /> inspection were Jeff Keller, permittee, and Ben Langenfeld, the consultant working on AM04 for APC. After signing in, <br /> DRMS accompanied APC personnel to verify the proposed permit area and boundary markers. Most of the permit area <br /> is defined by preexisting fences along the property lines. <br /> DRMS also spent time on-site discussing adequacy issues that were identified in the AM04 submittal. Primary issues <br /> discussed were the comment letter provided by the State Engineer's Office (SEO) regarding storm-water/groundwater <br /> on site, new annual reporting requirements to document high-wall conformance and stability, and the need to identify <br /> and obtain structure agreements for the Sherriff and Fire Department structures located in the far SE corner of the <br /> permit area. <br /> No other issues were identified during this inspection. <br /> Inspection Contact Address <br /> Mark Martin <br /> Asphalt Paving Company <br /> 14802 W 44th Ave <br /> Golden,CO 80403 <br /> Page 2 of 3 <br />
