PERMIT#: M-2015-030 <br /> INSPECTOR'S INITIALS: ECS <br /> INSPECTION DATE: February 12,2019 <br /> OBSERVATIONS <br /> This was a routine monitoring inspection conducted by Eric Scott of CDRMS. DRMS met with Greg Geras and <br /> David King of Asphalt Specialties at the site at approximately 1000. The site entrance is located on the NW <br /> corner of the intersection of Hwy 85 and County Rd 8 in Weld County, and the required identification sign was <br /> posted and visible (Photo 1). <br /> Mining/backfilling was taking place during this inspection in the north area of Cell 4, the concrete plant is <br /> operational in Cell 1, the gravel plant was operating in Cell 6, and the concrete and asphalt recycling area is <br /> active in the SW corner of Cell 4. Cells 2, and 3 are largely unaffected at this time by site activities. <br /> It is worth noting that the boundary markers placed at this site delineate the affected area, not the permit <br /> boundary. The affected area was well marked at the time of this inspection by existing fences and signs. It was <br /> also noted that the gas/oil wells and flow lines currently shown on the site map have been plugged and <br /> abandoned, but are still present. The site maps should be updated when these features are removed. <br /> The only issue noted during the inspection was that a shallow stripped area behind the topsoil pile in the SW <br /> corner of Cell 4 appears to either be retaining precipitation or exposing groundwater. This area will either need <br /> to be backfilled to eliminate groundwater exposure, or pumped to discharge impounded storm water. In <br /> addition, mowing and weed control should be implemented within the affected area when and where necessary <br /> to control noxious species (cheat-grass, knapweed, thistle, etc.) until the reclamation vegetation is fully <br /> established. <br /> No other issues or concerns were noted during this inspection. <br /> Inspection Contact Address <br /> Greg Geras <br /> Asphalt Specialties Co., Inc. <br /> 10100 Dallas St. <br /> Henderson,CO 80640 <br /> Page 2 of 3 <br />
