At the <br />conclusion of the review, the Commissioners unanimously voted to approve the above referenced application with <br />the following conditions: <br />1. Applicant must obtain all required state permits prior to issuance of this CUP. <br />2. Applicant must obtain all required federal permits prior to issuance of this CUP. <br />3. The applicant will need to provide adequate assurance that no part of the operation or any storage <br />will be placed in the Special Flood Hazard Area, or SFHA, without first obtaining the appropriate <br />permits. Adequate assurance may include: documentation of the SFHA Boundary, the Base Flood <br />Elevation of the 1% flood event, and the adjacent grades of any areas that might be used for <br />equipment or materials storage. Any materials that are stored in the SHFA shall require submittal of <br />a "No Rise Certificate". <br />4. Applicant shall submit a detailed site plan for each phase of the proposed excavation, showing the <br />location of every proposed structure, whether temporary or permanent, parking areas, and the <br />location of every stockpile. <br />5. Additional possible information required: Soils report, Letter regarding impact to wildlife from <br />Colorado Parks and Wildlife, Mining plan, Reclamation plan, Re -vegetation report. <br />6. A vehicle circulation plan showing truck ingress and egress from CR611 should be provided by the <br />applicant; access to the site shall only be provided from State Highway 69 to CR611. <br />7. The CUP shall expire in 2030 with further extensions subject to staff compliance review every 10 <br />years thereafter in lieu of a PC/BOCC review, except in the event of non-compliance. <br />8. A weed mitigation plan is required during operation of the facility. <br />9. Applicant shall return the county road to the same condition they found it. Dust control shall be <br />applied from State Highway 69 along CR611 to the site entrance on an annual basis. <br />10.
