Actions taken to respond to and correct the emergency situation or condition <br /> • At approximately 11:00 am on December 28, 2018, an error of the level indicator of the PO <br /> Thickener Tank caused approximately 500 gallons of process water to exit primary <br /> containment. Process water consists of fresh water and sediment originating from the milling <br /> and filtering processes that occur in the HGM. No cyanide is utilized anywhere in the mill <br /> circuit,therefore there is no risk that the process water contains cyanide. <br /> • Upon discovery, the overflow pumps in the PO Thickener Tank were manually activated so <br /> as to bypass the malfunctioning tank level indicator. <br /> • A skid steer was used to over-excavate the spill-impacted soil. <br /> • The excavated soil was transferred to the ore feed pile to be re-processed in the HGM. <br /> Approximately 7 cubic yards of soil was excavated in the cleanup efforts.
