Environmental Protection Specialist " Colorado Department of Natural Resources Division of Reclamation,Mining and Safety Office of Mined Land Reclamation 1313 Sherman Street, Room 215 Denver, Colorado 80203 Re: Permit No. M-1980-244; Cripple Creek& Victor Gold Mining Company; Cresson Project; Written Notification—Process Water Spill,November 30,2018 4 Dear Mr. Cazier: In accordance with Rule 8, Section 8.2.3 of the Mineral Rules And Regulations of the Colorado Mined Land Reclamation Board For Hard Rock, Metal, And Designated Mining Operations (Rules), Newmont Mining Corporation - Cripple Creek and Victor Gold Mining Company (CC&V) hereby provides the Division of Reclamation, Mining and Safety (Division) with a formal report regarding the process water spill that occurred at the High Grade Mill(HGM) on November 30,2018. The following points address the requirements of Rule 8, section 8.2.3: a. Actions taken to respond to and correct the emergency situation or condition • At approximately 8:30 pm on November 30, 2018, a process water conveyance pipeline separated. The separation occurred on the West side of the HGM within the leach tank containment, which contained the majority of the water. Approximately 75 gallons of process water overtopped primary containment, but remained on the HGM pad. The process water consists of fresh water and sediment originating from the milling and filtering processes that occur in the HGM. No cyanide is utilized anywhere in the mill circuit, therefore there is no risk that the process water contains cyanide. • Upon discovery, the leaking conveyance pipeline was isolated and the separated pipe was repaired. • A skid steer was used to over-excavate the spill-impacted soil. • The excavated soil was transferred to the ore feed pile to be re-processed in the HGM. Approximately 3 cubic yards of soil was excavated in the cleanup efforts. Clean gravel was placed in the over-excavated area.
